Mitra Clip is an interventional procedure used in mitral regurgitation patients who are surgically inoperable or are at high surgical risk. However, there is a failure rate associated with this procedure and in some instances no surgical intervention can be offered due to the inoperability risk. We report a case of failed Mitra Clip in a patient that was originally considered to be high surgical risk due to frailty that later required a double valve surgery for failed Mitra Clip within 1 year of the procedure. Although her surgical risk was even higher at the time of surgery, the patient tolerated the surgical procedure well with no complication. This case highlights the importance of properly defining patients at high surgical risk at the first instance as subsequent surgery can be performed with excellent outcome.
